02Congenital long QT and the Class drugs

Congenital long QT is the other named avoid. A cousin story is a reason to ask for an ECG, not a reason to tick 10 mg and hope.
Nitrates and riociguat remain class contraindications. They do not become optional because this memo is about QT. A man can have both problems.
Switching to tadalafil or sildenafil to 'skip QT' is a clinician's call after they have the rhythm file. It is not a pair-sheet self-serve. Open those dockets as study files, then go to a visit.

03What a Levitra 2.5-10 request has to survive first

04Macrolides and other QT stackers
Clarithromycin sits on the strong-inhibitor list that caps vardenafil at 2.5 mg in 24 hours. Erythromycin is a moderate CYP3A4 inhibitor that still needs a dose look. Azithromycin on this site has its own QT flag on a different docket. Do not assume a Z-Pak is invisible.
A postmarketing pairing of vardenafil with another QT-active drug showed an additive effect. Stacking 'just one more interval drug' is how a 10 mg usual start becomes a bad idea.
This memo will not list every QT-prolonging medicine in a textbook. It will say: write the names. Do not write 'heart pills.'
If the remaining question is dinner versus a 60-minute peak, you are in the wrong file. Use the onset file after QT is clear.
05The 80 mg QT arm exists because ritonavir can get you there
That supratherapeutic dose was four times the highest recommended tablet. It was chosen because it covers concentrations seen when 5 mg vardenafil sits with ritonavir 600 mg twice daily. The labeled answer is 2.5 mg and then 72 hours empty, not a hopeful 10 mg because QT 'looked fine last year.'
Cobicistat uses the same 2.5 mg / 72-hour cap. The 400 mg ketoconazole group uses 2.5 mg / 24 hours. Those people do not get a usual 10 mg start while someone 'watches the ECG.'
Heart-rate rises in the 59-man study were small, about 5 beats per minute at 10 mg versus placebo. Small is not irrelevant on sotalol.
Child-Pugh C still means do not use vardenafil, QT or not. Child-Pugh B starts at 5 mg and stops at 10 mg.
If the remaining question is a fast peak after QT is clean, leave this memo and open the onset file. Do not bargain with 20 mg here.
06A Levitra form that is worth sending
Write amiodarone, sotalol, quinidine, or procainamide if they are on the box. Write a family long-QT story even if you have never been checked. Write nitrates and riociguat. Write ritonavir or cobicistat. Then write 2.5-10 mg, not 20 mg, and only if those lines are clean.
Congenital QT and Class IA or III drugs are avoid-use. Two-point-five milligrams does not punch a hole in that line. A year-old normal tracing does not delete sotalol.
The 80 mg QT arm exists because ritonavir can push concentrations there. The labeled answer on ritonavir is 2.5 mg every 72 hours. A shop that still offers 10 mg after that tick is not reading the label.

08Why 2.5-10 is the range this memo will discuss
Ten milligrams is the usual labeled start and was the therapeutic arm in the QT study. Two-point-five is the CYP3A4 floor (ritonavir and cobicistat: 2.5 mg / 72 hours). Twenty milligrams is a labeled maximum this memo will not open on while QT is unanswered.
The 80 mg QT arm was four times the highest recommended tablet, chosen because it covers concentrations seen with 5 mg vardenafil plus ritonavir. That is why QT and CYP stories travel together on this INN.
Age 65 and over and Child-Pugh B already start at 5 mg. Those starts do not cancel an amiodarone stop.
